A factory warranty runs out. An auto protection plan picks up where it stopped: when a covered part fails, the repair goes through the plan instead of straight to you.
New vehicles may be covered by the manufacturer's warranty, but once that has run out, you are responsible for any breakdowns that might occur.
That is the whole question behind a vehicle service contract, which is what an extended warranty for a used car actually is. Once the manufacturer stops paying for repairs, somebody else starts, and it is either you on the day or a plan you added when you bought the car.
There are four levels, starting with the powertrain and going up to a top level written like a manufacturer's factory warranty.
Level 1 of 4
The powertrain warranty a used car buyer usually has in mind, covering the parts that cost the most to put right: engine internals, transmission and transfer case, drive axle, the cooling system, and select seals and gaskets. Towing is included, and roadside assistance can be added.
Level 2 of 4
Everything in Essential, plus the systems that tend to go next: air conditioning, electrical, fuel, brakes and ABS, suspension, sensors, turbo or supercharger, fluids and diagnostic time. Roadside assistance is included from this level up, with a rental car while yours is in the shop.
Level 3 of 4
Everything in Enhanced, widened: steering, broader electrical and sensor cover, and interior and exterior components. It suits a driver who wants more of the car covered without going to the top level.
Level 4 of 4
The top level, written the way a factory warranty is written: covered unless the contract lists it as excluded, rather than covered only if it is listed. A wrap version covers the powertrain while a factory warranty is still running, and an extension takes cover out to 100,000 miles.
Every level lets you use any licensed repair facility in the United States or Canada. You are not sent to one shop, and you are not left looking for one on a Sunday.
